Thioridazine (thioridazine hydrochloride)

BOXED WARNING

Prolongation of QTc interval reported in a dose-related manner. Associated with torsades de pointes and sudden death; reserve for patients who fail to respond to or cannot tolerate other antipsychotics. Increased risk of death in elderly with dementia-related psychosis; not approved for treatment of dementia-related psychosis.

Management of schizophrenia in patients not responsive to or intolerant to other antipsychotics.

ADULT DOSAGE

Schizophrenia

Management of Patients Who Fail to Respond Adequately to Treatment w/ Other Antipsychotic Drugs:
Initial: 50-100mg tid
Usual: 200-800mg/day given bid-qid
Max: 800mg/day

Once effective control of symptoms has been achieved, may reduce dose gradually to determine minimum maint dose

CONTRAINDICATIONS

Severe CNS depression, comatose states, severe hyper-/hypotensive heart disease. Drugs that prolong QTc interval, congenital long QT syndrome, history of cardiac arrhythmias, drugs that inhibit CYP2D6 (eg, fluoxetine, paroxetine), certain other drugs (eg, fluvoxamine, propranolol, pindolol), patients with reduced activity of CYP2D6.

WARNINGS/PRECAUTIONS

Efficacy in treatment refractory schizophrenia is unknown. Do not give with QTc interval >450 msec; d/c with QTc interval >500 msec. Perform baseline ECG and K+ levels; consider Holter monitoring if torsades de pointes symptoms develop. Hypotension, rarely resulting in cardiac arrest, reported. May develop tardive dyskinesia (TD); consider d/c if signs/symptoms appear. Neuroleptic malignant syndrome (NMS) and convulsive seizures reported. Agranulocytosis, leukopenia, neutropenia reported; d/c with severe neutropenia. Larger than recommended doses resulted in pigmentary retinopathy. Hyperprolactinemia, galactorrhea, amenorrhea, gynecomastia, and impotence reported. May impair mental/physical abilities.

ADVERSE REACTIONS

TD, ECG changes, torsades de pointes, drowsiness, dry mouth, blurred vision, peripheral edema, galactorrhea, N/V, gynecomastia, impotence, constipation, diarrhea.

DRUG INTERACTIONS

See Contraindications. May potentiate CNS depressants (eg, alcohol, anesthetics, barbiturates, narcotics, opiates, other psychoactive drugs), atropine, and phosphorus insecticides. Severe respiratory depression and respiratory arrest reported with high dose of barbiturate. Paradoxical vasodilation with mixed α and β adrenergic agonist vasopressors (eg, epinephrine); avoid use.

PREGNANCY AND LACTATION

Safety in pregnancy and nursing not known.

MECHANISM OF ACTION

Phenothiazine; associated with minimal extrapyramidal stimulation.

ASSESSMENT

Before treatment, strongly recommend at least 2 trials, each with a different antipsychotic, at adequate dose at adequate duration. Assess for reduced activity of CYP2D6, history of cardiac arrhythmias, congenital long QT syndrome, epilepsy, severe CNS depression, comatose states, hyper-/hypotensive heart disease, HTN, previously detected breast cancer, low WBC, history of drug-induced leukopenia/neutropenia, possible drug interactions. Normalize serum K+ and perform ECG before initiating treatment.

MONITORING

Periodically monitor ECGs and serum K+; consider Holter monitoring if symptoms of torsades de pointes occur. Monitor for signs/symptoms of TD, NMS, CNS depression, pigmentary retinopathy and severe neutropenia. Monitor CBC and prolactin levels.

PATIENT COUNSELING

Inform about risk of potentially fatal heart rhythm disturbances. Advise to inform physician if taking any other medications. Counsel about the risk of TD with chronic use and other side effects. Avoid hazardous tasks.
